间接测热法在脓毒症患者能量代谢的研究进展

间接测热法在脓毒症患者能量代谢的研究进展

摘要:据2016年第三次国际会议共识,脓毒症定义为“宿主对感染反应失调引起的危及生命的器官功能障碍”,新定义相比旧定义,不仅阐明了脓毒症存在器官功能障碍,还说明严重的代谢紊乱。研究显示,脓毒症患者高代谢、机体的营养需求增加,其营养状态与疾病进展、治疗和预后密切相关,对于该类患者的营养支持是我们研究的重点。既往多采用 Harris-Benedict公式评估患者的能量代谢,因脓毒症患者代谢紊乱,实际能量消耗与预测公式计算值存在偏差,据此对患者进行营养支持可能会造成营养不足或营养过度。间接测热法是测定患者能量消耗的“金标准”,其通过测定耗氧量、二氧化碳产生量以及静息能量消耗,反映机体的代谢状态,为脓毒症患者存在的代谢问题提供重要信息。本文综述间接测热法在脓毒症患者中的研究进展、未来应用与探索,讨论面临的挑战,旨在为患者的治疗和早期诊断提供方向。
