25羟维生素D、BMI与强直性脊柱炎病情活动的相关性研究

摘要：目的：探讨强直性脊柱炎（AS）患者的25羟维生素D、体重指数（BMI）与疾病活动性之间的相关性，为优化疾病管理策略提供理论依据。方法：回顾性收集2019年9月30日至2025年1月31日于承德医学院附属医院风湿免疫科收治的132例AS患者的病历资料，评估患者的Bath 强直性脊柱炎疾病活动性指数（BASDAI）、Bath强直性脊柱炎功能指数（BASFI）评分、体重指数（BMI），检测炎症指标、25羟维生素D（25 VitD）指标。结果：BMI≥24 kg/m²的超重及肥胖患者25羟维生素D水平显著低于BMI正常的患者，而炎症指标、及BASDAI、BASFI评分显著高于正常BMI组（p<0.05），经积极治疗后，超重及肥胖的患者炎症指标及活动评分的下降低于BMI正常患者（p<0.05）。BMI与25羟维生素D水平与AS患者的炎症指标和BASDAI、BASFI评分具有显著相关性（p<0.05）。结论：25羟维生素D水平降低和BMI升高与AS疾病活动度密切相关，二者可能通过协同作用加重炎症。25羟维生素D对疾病活动性的独立预测作用，AS患者尤其是超重及肥胖的患者需及时补充维生素D、积极干预体重，改善AS的病情。
