Abstract:Lung cancer is one of the malignant tumors with the highest incidence and mortality rates worldwide, among which approximately 85% are non-small cell lung cancer (NSCLC). Due to the lack of specific clinical manifestations in the early stage of NSCLC, most patients are already at an advanced stage at the time of diagnosis, missing the optimal therapeutic window and resulting in a poor overall prognosis. Exosomes are basically nano-sized extracellular vesicles that can not only pack a wide variety of bioactive molecules inside their lumen, but also display these molecules right on their membrane surface. They are, by nature, tools for shuttling messages between cells, and they can influence and modulate tumor initiation and progression through many different pathways. A growing body of research has now demonstrated that exosome-carried microRNAs (miRNAs) are involved in regulating a whole range of processes in non-small cell lung cancer cells, including invasion, metastasis, angiogenesis, and drug resistance. This review carefully sorts through what exosomal miRNAs actually do in non-small cell lung cancer and the molecular mechanisms behind these processes, with the aim of finding new clues and pointing to new directions—ones that may well provide quite useful ideas for identifying prognostic and diagnostic biomarkers.
